Youth on Track
Targeted support for young people at risk of ongoing involvement with the justice system.


Is this the right support?
This program may be right if:
- a young person is at risk of entering the criminal justice system
- a young person has been referred following police contact or legal concerns
- there are challenges affecting behaviour, relationships or school engagement
- a young person would benefit from structured support and case management
Referrals to Youth on Track are usually made through NSW Police.
How it works
- Referral from NSW Police
- Intake and assessment
- Individual case management and support
- Development of goals and support plans
Support may include:
- mentoring and case work support
- support with education and training
- referrals to community services
- family engagement and support
- group programs to build skills and community engagement
Who this is usually suitable for
- Young people aged 10–17
- Young people identified as being at risk of further involvement with the justice system
- Young people referred by NSW Police
Participation is voluntary but requires engagement with the program.
Getting Started
Young people are referred through NSW Police.
Once referred:
- our team will make contact
- we will complete an intake and assessment
- the young person and family will be supported to develop a plan

Support typically runs between 3–12 months, depending on individual needs.
